Data Protection Complaint Form

Introduction:

You can use this form to make a complaint to Deloitte if you:

  • Are dissatisfied with your response to a Data Subject Rights Request;
  • Have questions or concerns about how your personal data has been handled;
  • Have concerns about potential unnecessary or excessive data processing;
  • Consider there to be a perceived breach of your rights and freedoms in accordance with applicable data protection laws; and
  • Have any other issue that pertains to Deloitte’s processing of your personal data.

All information provided will be treated confidentially and used solely for the purpose of managing and responding to your complaint. Your submission will be shared with the Data Protection Officer to review and respond; they may need to communicate with other areas of Deloitte to make enquiries.

You will receive an initial response via email and within 30 days from the date of your submission, in line with applicable data protection laws.

Please provide your contact information so we can communicate with you about your complaint.

Section 2: Details of the Complaint

Please provide specific details about your data protection complaint. Should your complaint relate to a Data Subject Access Request, we encourage you to review correspondence and materials sent to you by Deloitte in the first instance. If your complaint relates to a suspected data breach, please also complete Section 3.

(e.g., issues with a Data Subject Rights Request such as an incomplete response from Deloitte, data not deleted, excessive data collection, lack of transparency)

Section 3: If your complaint relates to a data breach
Please leave blank if not applicable. 

(e.g., your name, address, email, financial details, account information, etc.)

(e.g., have you experienced financial loss, identity theft concerns, emotional distress)

(e.g. reported this through other channels or to the regulator?)

 

Section 4: Supporting Information

Please attach any documents or evidence that support your complaint, if relevant. Ensure that you only include the minimum amount of data required for this purpose, and not include sensitive data, special category data, client data or business confidential data.

Examples of supporting evidence include emails, letters, screenshots, call logs, internal memos, etc.
