While the technology worked as intended, its design, assumptions, and decision logic were poorly understood by the business and lacked clear, audit appropriate documentation for the external financial auditor to rely upon in completing their work.
As a result, there was a significant likelihood that the controls in a financially significant process would fail, thus having material impacts on the audit and the opinion on controls.
