The company must be privately held. This includes companies whose voting stock is owned by families, employees (including ESOPs), private equity, venture capital, or individual owners. Companies that are publicly listed on an exchange or actively traded over the counter are not eligible.

Companies are ranked based on self-reported annual gross revenue from their most recent year-end financial statements. If you are a parent company, please provide a consolidated revenue figure that includes all subsidiaries. Separate forms should not be submitted for individual subsidiaries.
