1. When do I need to file my Opt In or Opt Out form by?
If you were a resident of British Columbia on February 14, 1997 you are not required to submit an Opt In Form if you want to take part in the Settlement.
If you received your breast implant(s) in Canada, but outside of Quebec or Ontario or lived in Canada anywhere other than in Ontario, Quebec or British Columbia on February 14, 1997, and you want to make a claim in this Settlement, you must submit an Opt In Form to the Claim Administrator on or before December 2, 2005. If you do not want to participate in this Settlement you should do nothing.
If you were a resident of British Columbia on February 14, 1997 and you do not want to participate in this Settlement, so that you can pursue individual litigation, you must submit an Opt Out Form on or before December 2, 2005.
2. If I do not submit an Opt In form by December 2, 2005 will I still be entitled to receive compensation?
If you were a resident of British Columbia on February 14, 1997 you are not required to submit an Opt In form.
For all others who are eligible to be claimants and you want to take part in the Settlement you must submit an Opt In Form by December 2, 2005 or you will not be eligible to receive compensation.
3. What do I need to submit?
• Opt In or Opt Out Form (by December 2, 2005);
• Claim Form (by February 1, 2006);
• Release (by February 1, 2006);
• Product Identification Documentation (by February 1, 2006); and
• If you are making an Injury Claim - Supporting Medical Documentation (by February 1, 2006).
4. Do I have to submit the original medical records or are copies OK?
You may submit a copy of the medical records. They do not have to be the originals. Please ensure the copies are legible or the Claims Administrator may not be able to accept your claim.

6. How much compensation am I entitled to receive?
We are unable to advise at this time how much each claimant will be receiving. There is a pool of money which is to be distributed to all claimants on the basis of each claimant's Cumulative Unit Score as detailed in Exhibit I of the Settlement – Compensation Schedule. When the Cumulative Unit Score for all claimants are known, the available funds will be split based on your Score Ratio.
7. How do I change my address & contact details?
To change your details you must provide a written notice to the Claims Administrator providing the following information:
• BC Identification Number
• What changes are to be made to your contact details
The notice must be signed by the claimant or authorized representative for the claimant and mailed or faxed to the claims facility.
A form entitled Change of Address Form is available on the Forms page.
8. How do I inform you of a change of lawyer?
To change your legal representative information you must provide a written notice to the Claims Administrator providing the following information:
• BC Identification Number
• What changes are to be made to your legal representative details
The notice must be signed by the claimant or authorized representative for the claimant and mailed or faxed to the claims facility.
