If you lived in British Columbia on February 14, 1997 and you want to make a claim in this Settlement, you must submit a Claim Form, with supporting documentation, to the Claims Administrator on or before February 1, 2006. If you do not want to participate in this Settlement, so that you can pursue individual litigation, you must submit an Opt Out Form on or before December 2, 2005.
If you received your breast implant(s) in Canada, but outside of Quebec or Ontario or lived in Canada anywhere other than in Ontario, Quebec or British Columbia on February 14, 1997, and you want to make a claim in this Settlement, you must submit an Opt In Form to the Claim Administrator on or before December 2, 2005, followed by a Claim Form, with supporting documentation, on or before February 1, 2006. If you do not want to participate in this Settlement you should do nothing.
The additional supporting documentation that needs to be submitted with the Claim Form are Product Identification, to identify the manufacturer of your breast implant(s), and Supporting Medical Documentation, if you are making an Injury Claim.
In order to be eligible for compensation under the Settlement, your documents must be postmarked on or before December 2, 2005 for the Opt In / Opt Out forms and February 1, 2006 for the Claim Form and supporting documentation.
BC Identification Number
The Claims Facility will assign each claimant a BC Identification Number. This unique number must be provided on all correspondence and communication, whether by email, telephone or fax to the Claims Facility.
If you printed the Forms directly from this website and did not receive a BC Identification Number, this is fine. The Claims Facility will provide a BC Identification Number in its next communication to you.
